Deliver a patient's reported outcome using a QR code

1. Navigate to the Patient's record.

2. Click Forms.

3. Add a PRO or go to an existing PRO.

4. Click the clipboard icon ().

5. Select QR code.

Then head over to your tablet and follow these instructions:

6. Scan the QR code.

• If you’re using an Apple device, simply pull up the camera to scan the code.

• If you’re using a Samsung, download a free QR code reader. Like QR code reader. Then, open the QR reader and use the camera to scan the QR code using the app.

• If you’re using a Kindle, download a free QR code reader from the app store. Open the QR reader and use the camera to scan the QR code using the app.

*Note: The Kindle camera is not as powerful as Samsung or Apple so you may experience difficulty with scanning the QR code at times. If this occurs, please try scanning the QR code again. If you only see a product code, try scanning the QR code again.

7. Click the link on the tablet to open the assigned PRO.

8. Allow the patient to take the PRO and then click Complete.

9. Make sure you close the browser tab once the patient is done with the survey.
