If a patient has a preexisting form assigned to them you will not need to assign an additional form for the same region of injury.

Step 1: Select the Clipboard icon to the right of the form you wish to complete an additional time.
Step 2: Select the Fill out form option.
Step 3: Fill out the form and select the Complete option.
Step 4: You will now see the form saved under the Patient Response section under the expandable form menu. The form menu will list subsequent responses and generate a visible score graph.
