Forms & Surveys 101

Surveys are assigned to a patient via workflows that come preloaded on Keet Health. To send a form to a patient, you can either add it to their care plan or send it to them in an email through Keet.

Note: You cannot customize default forms in Keet.

  1. Form name
  2. Form schedule & frequency: when the form will be available in the patients account for them to complete
    • At Assignment One Time: Form is to be delivered at assignment via a workflow and will only be assigned one time
    • Select Days: Form is available for the patient to complete on the indicated days
  3. Form assignee: Keet Health and System indicate the form was assigned via a workflow
  4. Complete form
  5. Move/reorder forms
  6. Favorite form: once you favorite a form, you can search your favorites for easier access
  7. Edit form: edit the description, schedule, and/or frequency for the patient the form is assigned to
  8. Delete form

Add a Form to a Patient

While on the Forms tab, use the quick-search bar, input the form you wish to add, and select it from the populated search results.

It will be added to the patient's forms.
