If a PRO form was completed and submitted incorrectly, the form responses or the completion date can be edited. Additionally, if a duplicate form was inadvertently submitted, the duplicate form can be hidden.

Locate the patient in the Patient List.

Click the patient's name to access their account.

Navigate to the Forms tab.

Click the PRO name that includes the form to be edited.

Locate the form submission to be edited and click ACTION.
- Follow the steps below for the corresponding edit you'd like to make.
Edit Form:
A form response can be edited if a response was submitted incorrectly. For example, if upon review of a completed form with a patient, the clinician identifies that the patient misunderstood a question and verbally clarifies the question and the patient's response. To edit a form response:
- Locate the patient in the Patient List.
- Click the patient's name to access their account.
- Navigate to the Forms tab.
- Click the PRO name that includes the form to be edited.
- Locate the form submission to be edited and click ACTION.
- Click Edit Form.
7. Locate the response to be edited.
8. Select the correct response.
9. Scroll to the bottom of the form and click Update.
Edit Completion Date:
If a form was captured on paper and input in the Keet on a different date from when the form was originally completed, the Form Completion Date can be edited to the original date it was collected. To edit the form completion date:

Locate the patient in the Patient List.

Click the patient's name to access their account.

Navigate to the Forms tab.

Click the PRO name that includes the form to be edited.

Locate the form submission to be edited and click ACTION.
- Click Edit Completion Date.
- Using the calendar, select the appropriate completion date.
- The edited completion date auto-saves upon selection and is verified by a "Completion Date Updated" pop-up in the top-right corner of the screen.
Hide Response
A form response can be hidden for any form that was completed and submitted inadvertently. For example, a patient completes the assigned PRO after they have been discharged. To hide a form response:

Locate the patient in the Patient List.

Click the patient's name to access their account.

Navigate to the Forms tab.

Click the PRO name that includes the form to be edited.

Locate the form submission to be edited and click ACTION.
- Click Hide Response.
- A pop-up verifies that the form has been hidden.
